Hi Russell,

I have not looked, but somehow your <form></form> is screwed up. Open the
page in DW and move the </form> (where it makes sense). If you are writing
it out with iHTML then you are probably writing it out in the wrong place.
Take the results of your served page and put that into DW and move the
</form> tag around there. This will tell you where you need to put the tag.
(it might not be where the </form> tag is, rather the <form>)
