We have done all sorts of processes in PhotoShop to reduce noise and
anitaliasing in order to reduce the final numer of points and lines. It has
helped greatly but the issue at hand is one of accuracy and not file size
etc.

What processing are you doing to the image *before* you try to vectorize it?
If you take it into Photoshop (or whatever) and posterise it down to
as few colours as you can, as well as removing any noise and possibly
sharpening it a bit, that should make the resulting vector image a
whole lot better.
You have to expect some cleanup no matter how you process it though ...
