If over the internet:

1) Movies take time to load. Are you waiting long enough?

2) MIME must be set correctly

3) As already pointed out, some servers are sensitive to file name case -- 

use all lower case consistently to avoid problems.

4) Mac IE 4.5 does not Load Movie correctly.
