try Turns - I know they've done this kind of thing for Onvocation.com in the
past

you need to talk to Silas Denyer: silas [dot] denyeratturns [dot] net (mailto:silas [dot] denyeratturns [dot] net)

hope this helps,

Objet : UKNM: online surveys

There were several posts a few days ago about online surveys, but
unfortunately I've deleted them all. I'm trying to find suppliers and
costings for a branded online survey (the sort of "click here for yes",
"click here for no" type). Never done this kind of thing before, but the
idea is to have the survey hosted on one or more major B2B sites - in your
collective experience, are a lot of sites happy to host this kind of thing?

Thanks in advance,

Fiona
