For a company that provides web-based and email-response questionnaires, try

CASTING VOTE - http://www.castingvote.com


They can set one up on your site in your look and feel in HTML and also

provide an email voting system in plain text or HTML that people can respond

to. It can be customised really easily too.


I think their prices are pretty good value (can't remember them off-hand!).
