Usually it's because the file isn't found properly. This could be due to a couple of reasons:
1) incorrect capitalization (MyFile.swf instead of myfile.swf)
2) loaded movie filename contains spaces (My File.swf instead of MyFile.swf)
3) you didn't upload the file to the server
