Install (or re-install) QT 4?
If that doesn't fix it:
Output to AVI (indeo 4.4 or below), then use QT Pro to convert to .mov.
Apple released indeo 4.4 for QT separatly originally, not sure if you
still need to download the codec. You may be requesting features (such
as streaming sound) that require a codec you don't have installed,
apple.com/quicktime covers these issues pretty well.
