Until Macromedia decides to keep it's long standing promise to release
the updated Flash4 format specs the best you can do is to NOT use the
information from the Macromedia site. It's quite 'buggy' or rather it is
incorrect.
If you want a rectified version go to openswf.org and download the docs
from there. This will save you hours of frustration.
As for your questions,
1. Info is incorrect. There is only one ColorTransform in each button
record.
2. The correct order of fields in a definebutton2 is
Header - RecordHeader - TagID 34
ButtonID - UI16 - ID
Flags - UI8 = 0 or 1 - Track as menu flag
Offset - UI16 - Offset to the first Button2ActionCondition (immediately
following the buttonrecords)
Buttons - ButtonRecord[one or more]
ButtonEndFlag - UI8 = 0
Button2ActionCondition :
ActionOffset - UI16
Condition - UI16
Actions [0 or more]
ActionEndFlag - UI8 = 0
