Rob,

Take a look at www.zoomerang.com, free and $200/yr versions (can use $200
version free for 30 days). Web based surveys, easy and fast to build,
excellent results analysis. I looked at several low cost web based survey
providers and zoomerang were the best.

Two drawbacks:
- Zoomerang logo on the entry, survey and exit pages, you can put your logo
there too
- Can't put a link from the survey exit page back to your site

Worth trying out.
