The reason the IE 3 wanted to install the ActiveX Control is because
it is set for a minimum of Flash 3 in the HTML. See the version # in the
codebase.

codebase="http://active.macromedia.com/flash2/cabs/swflash.cab#version=3,0,0,0"

The most probable reason why the installation failed is because the
browser doesn't have the Authenticode update. The module to test for
this problem in my AllFlash detection system will be completed this week
but in the meantime you can solve this problem by either installing the
Authenticode Update or by temporarily lowering the Security level to
None.
